We
welcome you for showing interest for
empanelling your hospital / nursing
home with our TPA Network of Medical
Service Providers. Please note the following
details to be part of our Network. |
| These
are the criteria laid down by the insurance
companies for the Medical Service Providers. |

| |
 |
Registered
as a Hospital / Nursing Home with the
local authorities and is under the supervision
of the registered and qualified medical
practitioner. |
| |
|
|
If
Hospital / Nursing home, it should comply
with minimum criteria as under |
|
- It should have at least 15 inpatient
beds.
- It should have fully equipped Operation
Theater of its own wherever surgical operations
are carried out.
- Fully qualified nursing staff under
its employment round the clock.
- Fully qualified doctor should be in
charge round the clock.

| We
require you to submit a set of documents given
below in zip format, duly completed in all
respects. Please follow the instructions carefully.
The documents are to be stamped on all pages
and duly signed with stamp. Please send us
the documents to our Corporate Office at Chennai
(address given hereinbelow) for scrutiny and
approval. |

note: Empanelment with Gokulam TPA Network
offers you just an opportunity for your business
to grow. It gives an identity to your hospital
in our Network as a preference with an intention
to provide wider options for the customer
to choose from. |

| Please
go through all the Terms and Conditions to
join our network. We will be happy to respond
to you within ten days or earlier on receipt
of the signed MOU. |
